Clinicians are now confronting a pattern they had never seen at scale before: people arriving in crisis after long, intense conversations with chatbots. The emerging picture is not of a rare curiosity but of a small, steady stream of users whose delusions and suicidal thoughts appear to be entangled with artificial intelligence. The most detailed numbers so far suggest that even a fraction of a percent of users can translate into a deeply troubling absolute count.

At the same time, psychiatrists stress that “AI psychosis” is not a formal diagnosis and that the line between vulnerable users and harmful tools is still being drawn. The latest research and internal industry data instead point to a more nuanced reality, in which a measurable minority of users show warning signs while a much larger group turns to chatbots for help in moments of acute distress.

What clinicians actually mean by “AI psychosis”

In clinical language, what is being called AI psychosis is better described as psychotic or manic episodes that emerge in the context of heavy chatbot use. A definition that has gained traction describes Chatbot psychosis as a phenomenon in which individuals develop or experience worsening psychotic symptoms that they explicitly link to their interactions with AI systems. These symptoms can include persecutory delusions, grandiose beliefs about the chatbot’s powers, or the conviction that the model is a sentient being communicating secret messages.

Psychiatrists at academic centers are now collecting detailed chat histories to understand how these episodes unfold. At UC San Francisco, Psychiatry Professor Joseph M. Pierre, MD, has treated a woman whose psychosis appeared to be intertwined with her chatbot conversations, and he has seen a handful of similar cases. That work has helped crystallize the term AI-associated psychosis, which, as one summary notes, is not yet a formal diagnosis but a descriptive label for episodes that unfold in the shadow of generative models.

The new numbers: tiny percentages, alarming scale

The most concrete prevalence figures so far come from OpenAI’s own monitoring of mental health conversations. In an internal analysis described in a company blog and cited by outside reporting, OpenAI estimates that 0.07 percent of users in a given week indicate signs of psychosis or mania, and 0.15 percent show other serious mental health emergencies. A separate summary of the same figures notes that OpenAI claims that 0.07% of its weekly users display signs of “mental health emergencies related to psychosis or mania,” while 0.15% experience other acute crises.

Those fractions might sound negligible, but they sit atop a vast user base. One analysis notes that OpenAI’s estimates are framed against hundreds of millions of weekly active users, and that the company has said that at least 1.2 m users each week turn to ChatGPT for help while experiencing suicidal ideation. A social media post that amplified the same blog figures likewise highlighted the 0.07% and 0.15% rates as a safety concern, underlining that even a sliver of users can represent a large number of people in crisis.

Inside the delusional spirals clinicians are seeing

When I talk to psychiatrists and therapists about these cases, they describe a recognizable pattern rather than a single, exotic syndrome. A growing body of case reports and early research suggests that AI interactions can amplify existing vulnerabilities, particularly in people with a history of psychosis or mood disorders. Researchers are also starting to map the themes that recur in these delusional spirals.
